    • Signing a Deed Poll Document?
    • When you order your Deed Poll from UK Deed Poll, the important sections are all pre-completed. This ensures that your Deed Poll is accurate (unlike some companies which ask you to complete your own form).

      All you need to do is sign and date the relevant sections, and your Deed Poll becomes a legal document ready for use.

      1.   Fill in the date in the last paragraph. This is the date that you want your name change to be Legal form.

      2.   It can be today's date, or a date in the future (by default our system displays the current year, if you want a different year, then let us know).

      3.   Sign the Deed Poll with your New Name on the dotted line above where your new name is printed.

      4.   Sign the Deed Poll with your Old Name on the dotted line above where your old name is printed.

      5.   Have your witness sign on the line labelled Witness, print their name and complete their address and occupation details. Your witness can be anyone over the age of 18, who is not related to you.

      6.   For a child's Change of Name a parent or a Legal guardian will have do all the above mentioned steps on behalf of the Child.
